Yes. Focused Health Care provides in-home clinical nursing across Brisbane. Depending on your needs, this may include wound care, medication administration, diabetes management, clinical monitoring, rehabilitation support or palliative nursing.
Depending on your care requirements and funding arrangements, our team may provide nursing, rehabilitation and practical support following hospital discharge. Private clinical nursing can also be arranged directly where appropriate.
Services may be accessed through Support at Home, eligible NDIS or DVA funding, or private payment. The appropriate pathway depends on your circumstances, eligibility, approved supports and the type of care you need.
