Who is eligible for Support at Home

What is the Support at Home program?

Support at Home is Australia’s new approach to aged care in the home. It brings together a range of supports under a single, flexible system designed to help older Australians maintain their independence for as long as possible – even if you are self-managed.

Instead of managing multiple providers or programs, you can receive all your services through one registered provider — making your care simpler, clearer and more coordinated.

Your dedicated care partner

You will have a consistent point of contact — your care partner — who helps you plan and manage your services, monitors your progress, and ensures your care remains aligned with your goals.

0% participant contribution for clinical supports

All clinical supports in the program are funded at 0% participant contribution. This means you can access nursing and allied health supports through Focused Health Care without out-of-pocket costs for these items.

What is the Support at Home program

Who is eligible for Support at Home?

You may be eligible if:

  • You are an older Australian needing support to remain independent
  • You have been assessed through My Aged Care and assigned a Support at Home classification
  • You require services such as clinical care, personal care, transport support, social engagement or assistance with daily tasks.

Focused Health Care works closely with individuals and families to help you understand eligibility and next steps.

Our Support at Home services

Support at Home groups services into three categories.

As an Aged Care registered provider, we deliver all three, guided by our commitment to compassionate care, and clinical leadership.

Clinical supports

Nurse-led care delivered in your home to support your health and wellbeing.

  • In-home nursing
  • Wound care
  • Diabetes support
  • Rehabilitation
  • Allied health needs

Clinical supports are funded at 0% participant contribution.

Independence supports

These services help you maintain routines, stay connected and feel confident at home.

These supports include:

  • Personal care
  • Medication support
  • Community engagement
  • Transport
  • Respite care.

Every day living supports

These services ensure your home environment remains safe, comfortable and supportive of your wellbeing.

This includes:

  • Domestic assistance
  • Home and garden maintenance
  • Meal preparation and shopping.

Short-term pathways

Support at Home includes three time-limited pathways for people who have specific or changing needs.
Focused Health Care is accredited to deliver all three pathways, using a coordinated, nurse-led approach.

Restorative care pathway

A structured, 16-week multidisciplinary program designed to help you regain function or maintain independence after an illness, injury or hospital stay.

Our team provides coordinated nursing, rehabilitation and allied health services to support your recovery goals.

End-of-life pathway

A dedicated 12-week pathway providing compassionate palliative care for individuals who wish to remain at home.

We support you and your family with holistic, nurse-led care that prioritises comfort, dignity and personalised support during this time.

Assistive technology and home modifications (AT-HM)

This pathway provides separate funding for equipment and modifications that support independence at home.

We help you navigate your AT-HM options, understand funding tiers, and access equipment such as mobility aids, bathroom modifications or assistive devices.
